Hello! Welcome to the West Park Counseling Ministry.

In order to receive counsel through the Counseling Ministry, a Request for Counseling form and a Personal Data Inventory must be filled out and turned in to the West Park Counseling Ministry. Once these forms are received, you will be contacted to set up an appointment.

Completion of Request For Counseling Form

Form may be submitted as follows:

1. Access the form online, fill it out online, and submit it online.
   
   
2. Access the form online, fill it out online, print it, and then either mail/deliver the printed and completed form to the church office (address below).

Should you come across any concerns regarding this process, the Request for Counseling form or the Personal Data Inventory, please call the church at 690.0031 and ask for the Counseling Ministry.
